You've already agreed to these terms in a previous consultation, so we've pre-checked and pre-signed them below — just review and submit.
1. Custom Formulation. I understand that the products I receive are custom-formulated to my specifications. Bliss Ascension does not carry pre-made stock. Each blend is created specifically for me based on the information I provide.
2. Accuracy of Information. I confirm that all health information, allergy disclosures, and personal details provided in this consultation are accurate and complete to the best of my knowledge.
3. Patch Test. I understand that essential oils are potent botanical extracts. I agree to perform a patch test on a small area of skin before applying any blend more broadly, and to discontinue use if irritation occurs.
4. Pregnancy & Medication. I understand that certain essential oils are contraindicated during pregnancy, breastfeeding, and when taking certain medications. I have disclosed all relevant conditions and agree to follow the guidance of my blender.
5. External Use Only. I understand that all The Aroma Bar products are for external use only. I will keep all products away from eyes, mucous membranes, and out of reach of children.
6. Release of Liability. I fully and irrevocably release Bliss Ascension, Kelly, and any associated practitioners from any and all claims, demands, damages, losses, costs, or liabilities of any nature — whether direct, indirect, or consequential — arising from the formulation, handling, or use of my custom blend. This release applies even in circumstances where the product is used correctly and all information has been disclosed truthfully. I understand that individual responses to botanical ingredients can be unpredictable and that Bliss Ascension cannot be held responsible for adverse reactions that occur despite reasonable care.
7. Not Medical Advice. I understand that aromatherapy is a complementary wellness practice. Nothing in this consultation or the products I receive constitutes medical diagnosis, treatment, or advice.
8. Refunds. I understand that because each blend is custom-formulated for me specifically, refunds are not offered once blending has begun. If I am dissatisfied, I agree to contact Kelly directly to resolve the issue.
7. Assumption of Personal Responsibility. I acknowledge that I am solely responsible for how I use my custom blend. I will read all usage guidance provided, conduct a patch test prior to full application, and consult a qualified medical practitioner if I have any concerns about interactions with my health conditions or medications. Bliss Ascension's role is that of a skilled artisan and wellness practitioner — not a medical professional — and no product or recommendation should be treated as medical advice.
8. Limitation of Claims. In the event of any dissatisfaction or adverse reaction, I agree to contact Bliss Ascension directly before pursuing any other course of action. I acknowledge that my recourse is limited to the value of the product purchased. I waive any right to claim damages beyond this amount, including but not limited to claims for emotional distress, consequential losses, or reputational harm.
9. POPIA. I consent to Bliss Ascension storing and using my personal information for the purpose of formulating and delivering my blend, and for contacting me regarding future orders and offerings. My information will not be shared with third parties.

Workshop Registration
A simple registration form for The Aroma Bar aromatherapy workshops. Please complete before attending.
This helps your blender keep you safe during the workshop.
1. Nature of the Workshop. This is a creative aromatherapy blending workshop. All products are blended by participants under guidance and are for personal use only.
2. Patch Test. I understand that essential oils are potent and may cause reactions. I agree to perform a patch test before applying any blend to larger areas of skin.
3. Allergies & Health. I confirm that I have disclosed all known allergies, sensitivities, medications, and health conditions relevant to the use of essential oils.
4. Pregnancy & Breastfeeding. I understand that certain oils are contraindicated during pregnancy. If applicable, I have disclosed my status and will follow the facilitator's guidance.
5. Assumption of Risk. I voluntarily participate and accept full responsibility for the products I blend and how I use them.
6. Release of Liability. I fully and irrevocably release Bliss Ascension, Kelly, We Move Up, and all associated facilitators, assistants, and venue holders from any and all claims, damages, losses, or liabilities — whether direct, indirect, or consequential — arising from my attendance at and participation in this workshop. This includes but is not limited to any adverse reaction to essential oils or carrier ingredients used during the event, whether or not I disclosed all relevant health information. I understand that the facilitator is a wellness practitioner, not a medical professional, and that participation is entirely at my own risk.
7. Not Medical Advice. Nothing shared during this workshop constitutes medical advice, diagnosis, or treatment. Aromatherapy is a complementary wellness practice. I will seek advice from a qualified medical practitioner for any health concerns before attending.
8. Personal Responsibility. I accept full personal responsibility for the blend I create during the workshop and any subsequent use thereof. The facilitator provides guidance only — the final product is made by me, for me. Bliss Ascension cannot be held liable for how I blend, store, or apply any product created during the session.
9. Limitation of Claims. I acknowledge that in the event of any dissatisfaction or adverse experience, my recourse is limited to the value of the workshop fee paid. I waive any right to pursue claims beyond this amount, including consequential, indirect, or punitive damages of any kind.
10. Photography. I consent to photographs being taken for use on Bliss Ascension's social media and marketing materials, unless I indicate otherwise to the facilitator on the day.
11. POPIA. My personal information is collected solely for the purpose of this event and will not be shared with third parties. I consent to being contacted by Bliss Ascension regarding future events and wellness offerings.
